I use adsense on my forums. I decided to try an additional 468x60 banner sitewide at the very top of the page in addition to the 728x90 which is lower down directly at the top of every topic and forum. This more than doubled the number of impressions. The wierd thing is this caused my adsense income to drop and then go back to normal after removing the 468x60. I never had a drop in income when adding adsense banners lower on the page.
The 728x90 was always going to have a better click through rate because of it's poisition being closer to the content people are reading compared to up at the top out of the way for the 468x60. My suspiscion is that the highest paying ads are shown first on the page and then in descending order across the multiple adsense units and by adding the 468 up at the top I moved the best paying ads away from the premium CTR position. So something to try out is make sure that the first adsense banner to load on your page is in the prime position for best CTR because this 1st banner is where the highest paying ads may be displayed.
I'm not totally sure about this and I'd be interested in any feedback if you have tried this.

Adding additional AdSense units doesn't necessarily generate more income, as you would think it might.
By adding further ads, you're giving people more things to click on. If your unit that loads second actually gets a higher CTR, which can happen, then this could result in a reduction in revenue because people will be clicking on lower-paying ads.
